How to get yours SUS Card
To get the SUS Card, you need to go to a Health Center or UBS – Única Básica de Saúde.
First present your identity documents such as:
CPF, RG, Driver's License CNH
Proof of residence such as a water or electricity bill

Connect SUS application
Conecte SUS is the Ministry of Health's application, which records everything about the patient in the SUS.
In other words, you can see information about the patient's care and hospitalizations.
In addition, you can check medications and tests carried out and schedule appointments.
It also allows you to consult all vaccines administered in public and private networks, and even Covid-19.
Conecte SUS also offers a list of services and consultations aimed at the Brazilian population, such as:
Health centers, hospitals, emergency services and popular pharmacies.
Furthermore, it also provides guidance on blood and organ donations.
The application exists mainly to computerize the patient's file and speed up care.

1- Install the Conecte SUS application to android or iOS;
2- Log in with your account on the Government Portal, accessing here;
3- On the screen authorizing the use of personal data, click “Authorize”;
4- Read and accept the Terms of Use by clicking “I accept the terms”;
5- Click on “Schedule”;
6- Click on the calendar, in the bottom right corner;
7- Choose your UBS health unit and one of the available dates.
If you do not have any health unit registered, the options will not be displayed and, therefore, it will not be possible to schedule an appointment.
Therefore, you will need to request registration of your data in the National Health Data Network.
In other words, this must be done directly at the basic health unit in your city.
You must ask the responsible agent to transfer your registration to the digital system.
How to log in to the gov.br portal
To use Conecte SUS, you mainly need to have an account on the government portal.
To access your account, do this:
1- Access the portal here;
2- Enter your CPF to create the account
3- Fill in the data according to the instructions;
4- Create a password.
Now you can use your SUS Card and schedule your appointments online.
