How to obtain the SUS Card for the first time

For obtain the SUS Card, it is necessary to present identity documents such as: CPF, ID, Driver's License CNH and also proof of residence such as a Water or Electricity bill at the Post or Health Center or Hospitals in your municipality.

How to obtain the SUS Digital Card

Once you have already registered with the SUS Card in the Unified Health System and already have your SUS Card, you can obtain it digitally, thus eliminating the need for a printed card if you need to use it.

You can check and save your phone number SUS Card by CPF easily on your cell phone or computer, through ConecteSUS Portal, following these steps:


1. Access the ConecteSUS Portal;

2. On the Activity Wall, click on the “Enter” button;

3. Enter your CPF and click “Next”;

4. Then, enter your gov.br portal password. This is the Brazilian government portal that brings together all public services for citizens.

If you don't remember your password, click on “I forgot my password” to recover it. If you have not yet registered with gov.br, go to the website and follow the instructions to register;

5. With your registration and password in order, log in to ConecteSUS;

6. Immediately after logging in, the website's main page (Home) will display in the top left corner the data of the registered person, in order: full name, CPF and social security number. Card SUS/CNS (National Health Card). The number of SUS Card It consists of 15 digits.

If the number of SUS Card does not appear on the main page, find the “My data” option in the menu on the left. After clicking on it, the following data will be displayed:

There is also the option to consult the number of the SUS card through the application ConnectSUS Citizen. The application is available for android and also iOs.

After installing it on your cell phone, access the application and follow these steps:

1. Click on the “More” option located in the menu at the bottom of the screen;

2. Then click on the “Access now” button to access the services of the SUS;

3. To proceed, enter your access data;

4. The holder's data card must be displayed on the main screen, but if it is not possible to view it, find the “My data” option in the menu and click on it, after clicking, the card holder's information will be displayed. card.
